Fill out the form below to enroll your horse(s) in our 2025 MEH Wellness Plan Program. Please provide all relevant contact information, and be sure to specify which level of care you have
chosen for each horse in the field provided (ex: Stormy - Bronze, Chance - Gold). 2025 Wellness Plan descriptions and the 2025 Wellness Plan Terms and Conditions are included at the bottom
of this page for reference and are available together in a printable PDF format under the "Wellness Plans" tab above.

2025 Equine Wellness Plan Terms and Conditions
Basics
- Horses enrolled in an Equine Wellness Plan are entitled to one spring and one fall
wellness farm visit in the 12-month period following enrollment. The specific wellness services included in each visit vary by plan and are detailed in the charts above. Clients wishing to
consolidate the Bronze Level Wellness Plan into a single annual visit can do so, but more than two annual wellness visits will not be accomodated with any plan.
- Discounts offered through the Equine Wellness Plan program can only be applied to the enrolled horse.
- Wellness appointments or specific wellness services that are not utilized during the 12-month enrollment term are non-refundable, cannot
be substituted, and do not roll over.
- A horse's wellness level cannot be changed during the enrollment term, but may be changed the following enrollment year.
- Add-On Services are available to help further customize plans but must be scheduled at the same time as a spring or fall wellness visit
in order to take advantage of discounted rates.
Payment
- Payment of the full plan cost will be due at the time of the first wellness visit. We
accept cash, checks, and the following credit cards: Visa, MasterCard, Discover, and American Express.
- The total cost for each wellness plan covers all listed services for one horse. It DOES NOT include farm call
charges, fees for Add-On Services, or charges for any other services rendered at the time of the wellness appointment.
- Participants in the Equine Wellness Plan program can split call fees when appointments are scheduled together at the same
farm.
- If an enrolled horse is sold or dies while participating in the Equine Wellness Plan program, the remaining services can be transferred
to another horse.
Enrollment
- Enrollment for MEH Equine Wellness Plans will be open from February 1, 2025 to November 1,
2025.
- To enroll, please submit the electronic form on our website (www.manorequine.com).
- Once you have successfully enrolled your horse(s), you must call our main office to schedule wellness appointment(s) at your
convenience.
- Enrollment does not automatically carry over. Horses must be re-enrolled annually to continue taking advantage of the
program.
